Summary
A patient reported persistent facial pain for at least 19 months after receiving 7ml of Ellansé dermal fillers during one treatment session.
She reported high blood pressure, weight loss exceeding 20kg, mental health effects and job loss following the complications.
An interim committee restricted Dr Wong Kai Jie Isaac’s medical practice for up to 18 months from Aug 26, pending the conclusion of disciplinary proceedings.
SINGAPORE: A woman in Singapore suffered facial pain for at least 19 months after receiving dermal fillers from aesthetic doctor Wong Kai Jie Isaac, according to an official medical inquiry decision published on Oct 7.
The patient reported losing her job, developing high blood pressure and losing more than 20kg after changes in her appetite. She also experienced mental health problems and underwent nerve surgery to manage the persistent pain.
The Singapore Medical Council (SMC) raised concerns about the treatment and its aftermath. An Interim Orders Committee (IOC) subsequently imposed temporary restrictions on Dr Wong’s medical practice.
Dispute over the recommended filler dosage
The patient visited Dr Wong at The Artisan Clinic on April 1, 2023, for her first consultation. She agreed to undergo a procedure involving Ellansé dermal fillers.
According to the decision, the product packaging then stated a maximum annual dosage of 2ml. The patient reported receiving 7ml in one session, more than three times that amount.
Dr Wong disputed the claim that 2ml represented a clinically established maximum. He said the manufacturer had adopted that figure to meet Singapore’s regulatory requirements in 2016.
The manufacturer later changed the Singapore packaging to state a maximum annual dosage of 12ml. The change took effect in March 2026, after the treatment and complaint.
The SMC maintained that the label in force at the time mattered when assessing whether Dr Wong had exceeded the stated limit.
Complications led to repeated medical visits
After the procedure, the patient reported pain on the left side of her face. Dr Wong diagnosed a blood vessel blockage and administered hyaluronidase injections in an attempt to manage the complication.
The patient continued attending follow-up appointments until October 2024. She reported persistent pain and nerve-related symptoms, while scans showed severe narrowing or blockage involving facial arteries.
Dr Wong continued various treatments, including injections of hyaluronidase and steroids. Another doctor consulted during her treatment expressed uncertainty about whether further hyaluronidase injections would help.
Dr Wong disputed the SMC’s concerns about his treatment. He said hyaluronidase was intended to address the blood vessel blockage, rather than dissolve Ellansé itself.
Five other complaints raised during inquiry
The SMC disclosed five other complaints against Dr Wong before the IOC hearing. These included allegations involving excessive filler doses and adverse effects.
The doctor’s counsel argued that the other complaints didn’t involve the same blood vessel complication and hadn’t resulted in findings on their merits.
The IOC found a prima facie case and ruled that interim restrictions were necessary to protect the public while proceedings continued. The decision didn’t determine whether the allegations were ultimately proven.
Doctor barred from administering dermal fillers
From Aug 26, Dr Wong was prohibited from offering or administering dermal fillers for up to 18 months, or until the relevant proceedings concluded, whichever came first.
Dr Wong was also barred from treating filler-related complications, except in a life-threatening emergency when he was the only available doctor. Other conditions require him to follow approved dosage and product specifications, disclose his practice locations to the SMC and inform employers of the restrictions.
Cosmetic treatments may be elective, but their risks deserve serious attention. Patients should understand the proposed treatment, its known risks and the available options before consenting.
Doctors must also follow the applicable product guidance and ensure patients receive appropriate care when complications arise.
