The next transformation in healthcare will not happen inside the hospital alone. It will happen wherever the patient is. Given that management of chronic diseases requires constant care and specialist availability has become inconsistent, healthcare systems are under mounting capacity challenges. There is an increasing tendency towards moving care delivery outside clinical spaces into homes, communities and other locations beyond clinics. Technologies like remote monitoring, connectivity in medical devices, virtual care, cloud computing and AI enable us to extend clinical visibility outside hospitals and establish continuity in care delivery.
This is especially true in the case of India. According to the Union Ministry of Health and Family Welfare, the current doctor-patient ratio in India stands at around 1:811. Simultaneously, according to Philips’ Future Health Index 2026, 80% of the respondents who are healthcare professionals believe that AI will enhance access to quality care in rural regions of India. The opportunity is therefore not simply to take healthcare outside the hospital, but to use technology to extend clinical expertise, decision-making and monitoring closer to the patient.
Connected Devices evolving into Connected Care
Making this model work requires more than deploying connected devices. Healthcare enterprises need to build the technology layer that allows patient data, medical devices, clinical systems and intelligence to work together. The starting point is interoperability.
Connected devices, electronic health records, diagnostic platforms, and hospital information systems should be able to interact freely using standard protocols like FHIR/ HL7, instead of being isolated data sources. As per a report from Grant Thornton Bharat and the Association of Healthcare Providers (India), nearly 95% of the hospitals have adopted Hospital Information Systems; however, just 34% run on enterprise dashboards in real time.
The foundation of digitalization in healthcare has been laid already. The next challenge is connecting those digital systems so that information can move across the care journey and become actionable in real time.
Building the Technology Infrastructure for Continuous Care
This is where enterprise technology becomes critical. Cloud computing platforms have the potential to build the scalable infrastructure that is needed to aggregate the health data available from medical devices, hospitals, laboratories, and other healthcare applications that are used by the patients. Edge computing allows processing the data generated closer to its source to improve response times even in locations with poor connectivity.
On top of this infrastructure, AI and analytics can turn a stream of health data into clinical insights. Instead of burdening the clinician with having to monitor thousands of readings manually, AI could detect anomalies and patterns, and prioritize patients that might need more attention.
This changes the role of connected care from passive monitoring to predictive and proactive care.
The same technology can also support hospital operations. Real-time data from beds, medical equipment, operating rooms and patient flows can help healthcare organisations anticipate capacity constraints, optimise resource utilisation and improve response times. Connected care, therefore, is not only about keeping patients connected to clinicians; it is also about creating a more intelligent healthcare enterprise.
Making Technology Part of the Clinical Workflow
The next priority is ensuring that connected technology works within existing clinical workflows.
A connected blood-pressure monitor or wearable device has little utility if its information gets siloed in another application. Enterprises should focus on API-led integration, bidirectional data exchange and unified clinical dashboards that bring relevant information into the systems clinicians already use.
The goal must be simple and obvious: to have fewer disconnected applications, manual gathering of information and greater amounts of actionable insights.
This can be done even better with the help of AI technologies through summarizing historical information, alert triaging and highlighting any clinically important changes. This becomes all the more necessary due to the increasing adoption of connected health since a mere increase in the amount of data may make things worse for clinicians.
Building a Connected, Secure Care Ecosystem
The larger opportunity is to connect the entire care journey, from prevention and diagnosis to treatment, discharge, recovery and long-term disease management.
For healthcare enterprises, this means building connected technology ecosystems in which medical devices, EHRs, cloud and edge infrastructure, AI, analytics and patient-facing applications function as parts of one architecture. Security and privacy need to be embedded into this architecture through strong identity management, encryption, access controls and continuous monitoring as more systems and devices exchange sensitive health data.
The hospital, in this model, does not become less important. It becomes one intelligent node in a broader care network. The defining shift is therefore not simply that healthcare is moving beyond the hospital. Technology is making the patient journey continuously visible, connected and actionable.
For India, the next phase of healthcare transformation will depend on how effectively enterprises turn connectivity into intelligence, and intelligence into timely care.
The future of healthcare may still have hospitals at its centre. But the technology behind care will increasingly extend wherever the patient is.
