A new international study has found that residents of the Johannesburg–Pretoria region are breathing unsafe air on more than half of all days in a given year, with pollution levels routinely breaking South Africa's own national safety guidelines.

According to reports, the findings paint a troubling picture for one of Africa's most densely populated urban corridors, where millions of people go about their daily lives unaware of the invisible toll the air is taking on their health.

Across the country, fine particulate matter known as PM2.5 has become a growing concern. Exposure to these microscopic particles has increased by more than 5% since 2000, and scientists warn the consequences are far-reaching. Short-term exposure has been linked to higher rates of death from respiratory illness and non-communicable diseases. Over time, fine particles penetrate deep into the body, raising the risk of cardiovascular disease, lung cancer and complications during pregnancy.

The burden is not shared equally. Low-income households, children and older adults are hit hardest, particularly those living in informal settlements where coal and wood burning are everyday realities.

What is driving Gauteng's air crisis

Gauteng's air quality challenges stem from several overlapping sources. Coal-fired power and heavy industry in neighbouring regions send pollution drifting into the province, while dense traffic in Johannesburg and Pretoria adds to the load. Domestic burning of solid fuels in informal settlements contributes significantly, as do mining operations and local industrial activity. In winter, temperature inversions cause pollutants to become trapped close to the ground, making conditions especially dangerous during the coldest months.

Health advocates, including the South African Medical Research Council, have called for tougher enforcement of existing air quality regulations, a reduction in the country's dependence on coal, and closer integration between environmental and public health policy. The study's findings give renewed urgency to those calls, particularly as urban populations in Gauteng continue to grow.
