Interview with a certified addiction professional
In an exclusive interview with TUKO.co.ke, clinical psychologist and certified addiction professional Joyce Igogo opened up about drug addiction and how it can be differentiated from mental health challenges in order to ensure appropriate care. Take a look.
Q: Addiction may often be portrayed as simply a matter of poor choices or a lack of self-control. However, people struggling with substance use may also be dealing with trauma, anxiety, depression, low self-esteem, or other psychological difficulties that are not always visible to others. Drawing on your experience as a clinical psychologist and addiction professional who has worked with people in treatment, how are substance use problems and mental health difficulties related, and how can they be differentiated to ensure appropriate support?
Substance use problems could lead to mental health difficulties and the vice versa is true. Depression, anxiety, trauma, triggers cortisol-the stress hormone in the brain which causes emotional, physical and psychological discomfort for example insomnia, restlessness, racing or ruminating thoughts, irritability or low moods that pushes one to use substances in an attempt to solve the discomfort.
Long term substance use significantly changes the limbic and prefrontal cortex that triggers anxiety, depression, affects self-image, challenges mood regulation, judgement and insight.
Q: From the outside, the lives of young celebrities can appear extraordinarily privileged. Still, fame can also mean growing up under constant public scrutiny, intense career expectations, financial pressures and a loss of privacy, with very little room for making ordinary mistakes. Building on your experience of working with addiction and psychological wellbeing, what might make a young celebrity person more vulnerable to substance misuse?
In the psychosocial stages of development young adults are navigating intimacy versus isolation, being in the limelight can causes alienation as people view young celebrities as super human hence they lack meaningful deep relationships and could easily attract predators and opportunistic people.
Most careers require long hours, peak performance, huge financial obligation that are all stressful. The dance of finding genuine friendships and managing their careers is lonely, hurtful, stressful and can potentially lead to substance use in an attempt to feel better or trigger depression, anxiety and trauma due to the pressure and substance abuse.
Q: When the public hears that a famous person has entered rehabilitation, the story can quickly become about scandal and a damaged reputation, or about another celebrity 'falling off the rails'. From a clinical perspective, what are the common misconceptions about addiction when judging someone based primarily on their behaviour during active substance use, and what should families, friends, and the public understand about addiction as a health and psychological condition?
Addiction is widely interpreted as a moral failing or a lack of will power others see is as bewitchment while others view it as way of life for a certain gender, age group or career. As much as these are easy interpretations, Addiction is a disease of the brain just as the kidney and the liver gets unwell.
Substances used alter the brain mechanisms that alter the individual’s ability to think and interpret reality, make decisions, solve problems, regulate emotions, maintain proper hygiene, productivity in school, work and manage their relationship with friends and family.
Addiction majorly affects the ability to recognize that one is sick due to the substance use this is why the friends, family and colleagues will recognize it first and the individual will constantly deny the implications of their use leading to strained relationships and difficulty getting them help.
Q: Not everyone who uses a substance will develop an addiction, nor will every person struggling with substance use necessarily require the same form or intensity of treatment. Having worked in addiction treatment and studied what influences a person's motivation to engage with treatment, what signs should families look for when substance use has progressed from occasional to problematic, to the point where professional intervention or rehabilitation is necessary?
Substance use primarily starts as a choice and in the occasional phase one is in control of the use and all areas, excessive use of substances alter the brain mechanisms which leads to addiction. Addiction symptoms include:
- Occupationally - loss of employment, dropping out of school.
- Emotionally - irritability, anger outbursts, constant feelings of sadness, hopelessness, purposelessness, guilt, shame, regret.
- Psychologically - denial of substance abuse, self-hate, self-pity, low self-esteem, lack of motivation, suicidal attempts or talks
- Biologically - insomnia, poor appetite or emotional eating, significant weight changes, hyper acidity.
- Behaviorally - pathological lying, manipulation, conman ship, unmanageable debt, verbal, sexual, physical violence, poor hygiene and infidelity.
The family, at this point or the person abusing substances has to make a decision to get professional help as addiction has in the past led to death for example Prince.
Q: One of the most challenging aspects of addiction is what happens after rehabilitation, when a person must return to the same environments and emotional triggers that may have contributed to their substance use in the first place. Drawing on your experience in clinical practice and rehabilitation, what factors can lead to relapse even after successful treatment, and how can people recognise and manage the warning signs early on? Are there any support systems that can give someone the best chance of maintaining recovery over the long term?
Relapse is a process. The first stage is emotional relapse that looks like bottling up emotions, focusing on other people and their problems instead of self, poor self-care (physical and emotional), poor eating and sleeping habits, isolating from sober peers and family.
This then graduates to mental relapse which is constantly thinking of the people and places you used to use at, thinking of scenarios that is acceptable to use, planning to relapse only during holidays, hanging out with using buddies without using, psychological cravings and minimizing past consequences of use. Eventually it graduates to physical relapse which is when one goes back to use.
Its important to continuously see a therapist after treatment in the initial phase of recovery. There are solid AA and NA groups across the country that meet weekly that offer a safe space to share and seek support. Family understanding, empathy and support creates a safe space for their oved one to share their struggles and get help that boosts recovery in the long term.
