Among 187,341 American adults aged 50 and older followed for about a decade, those with an ADHD diagnosis in their medical record were later diagnosed with mild cognitive impairment or dementia 6.7% of the time, against 3.4% of everyone else (Bauer-Negrini G, Leffa DT, Ferreira PCL, et al. JAMA Psychiatry. Published online October 7, 2026).
The coverage turned that into a fourfold increase in dementia risk from ADHD. The study is a good one and the association in it is real. The number that traveled is the one that needs the most context, and the authors supply a fair amount of it themselves.
I treat adults with ADHD, and several have already asked me whether they should be frightened. The answer takes a few minutes, and most of it is encouraging.
Most of These Diagnoses Were Mild Cognitive Impairment
The participants entered the study at an average age of 56 and were followed roughly ten years. Dementia is uncommon in that window, and the results reflect it. The association was strongest for mild cognitive impairment, and the estimate for dementia alone rested on far fewer cases and a much wider margin of error.
Mild cognitive impairment at 58 is also a diagnosis that depends heavily on who is looking. Someone who has lived with ADHD for five decades and mentions misplaced paperwork, an appointment that slipped, and a sentence that went nowhere mid-meeting is describing an ordinary week. Recorded at 58 by a clinician meeting them for the first time, the same account can look like the beginning of something.
The Association Shrinks When the Two Diagnoses Are Pulled Apart
Many adults in this cohort were diagnosed with ADHD late, in midlife, often after going to a doctor about concentration. If a cognitive problem is what sent someone for the ADHD evaluation in the first place, the two diagnoses aren't independent events.
The authors tested this directly, requiring progressively longer gaps between the ADHD diagnosis and the cognitive one. Notably, the hazard ratio fell from 4.60 to 1.44 once eight years had to separate them, and it was no longer statistically significant at nine. Some of that is reduced statistical power, which the authors note. The size of the drop is still the most clinically useful thing in the paper.
The 2024 Lancet Commission on dementia prevention identified fourteen modifiable risk factors and estimated that eliminating all of them could prevent up to 45% of dementia worldwide (Livingston G, Huntley J, Liu KY, et al. Lancet. 2024;404:572-628).
Set that list beside the known course of untreated ADHD and the overlap is nearly complete. Smoking, excess alcohol, physical inactivity, obesity, hypertension, diabetes, depression, social isolation, traumatic brain injury, and less education are all more common in adults with ADHD. For three more of the fourteen, including untreated hearing loss and untreated vision loss, the risk factor is the unmanaged version, which is an adherence and follow-up problem of exactly the kind ADHD produces.
One detail makes that reading harder to dismiss. The adults with ADHD in this cohort were better educated than the comparison group and only slightly more likely to have smoked, so the study was run in a sample tilted against the very explanation the numbers kept pointing to, and the association still weakened substantially under adjustment.
Dementia Essential Reads
The authors reach for the same explanation. Their discussion proposes that ADHD increases exposure to cumulative risk factors including chronic stress, sleep disturbance, physical inactivity, poor diet quality, cardiometabolic risk, injuries, and substance use. They also caution that their genetic findings may reflect liability to ADHD and its life-course correlates rather than a direct effect on dementia biology.
Consistent with that, a 2022 study using genetic instruments to test causation found no effect of ADHD genetic liability on Alzheimer's disease (Pagoni P, Dardani C, Leppert B, et al. Transl Psychiatry. 2022;12:422). The estimate was almost exactly null, and it stayed null after accounting for educational attainment.
What I Tell a Worried 55-Year-Old
The response to this literature is unglamorous and almost entirely actionable. Get the ADHD treated. Whether treatment lowers dementia risk hasn't been tested, and the authors call it a key unanswered question, but the evidence that it lowers crash risk, injury risk, and all-cause mortality is solid, and treatment makes every item below easier to sustain. Get blood pressure, cholesterol, and blood sugar measured and treated. Stop smoking, with pharmacological help, which works better when the ADHD is treated at the same time. Keep alcohol well under guideline limits.
Have hearing and vision tested and corrected. Take head injury seriously, which means helmets, driving, and fall prevention later on. Get screened for sleep apnea, which overlaps with ADHD and is frequently mistaken for it, if there's snoring, daytime sleepiness, or inattention that doesn't respond to treatment. Treat depression. Keep moving and stay socially connected.
Every one of those is on the Lancet list, nearly all of them respond to ordinary medical care, and each is worth doing on its own evidence regardless of what it does to dementia risk.
Lifelong Inattention Is Not New Decline
The paper calls for cognitive monitoring guidelines in adults with ADHD. The case is reasonable, though screening instruments for early cognitive change were built for people whose baseline attention and processing speed are typical. Run a brief screen on a 60-year-old with lifelong ADHD and it will register inattention, slow timed performance, and weak delayed recall, which may well have been there at 25.
Given that, monitoring in this group has to be measured against a personal baseline rather than a population norm, which means the useful version starts in midlife, before anyone is worried, and repeats. What matters is change from your own baseline. A real change deserves formal neuropsychological testing. Depression, sleep apnea, alcohol, thyroid disease, and medication effects all produce cognitive complaints that mimic early decline and that respond to treatment.
Clinicians seeing midlife patients with ADHD should remain vigilant about blood pressure, smoking, alcohol, sleep, head injury, and mood at every visit, and equally careful about the inference a fourfold hazard ratio invites. The association is worth taking seriously. The burden it most likely reflects is one that medicine already knows how to treat.
I've written a fuller analysis of the study's methods and numbers, including the full adjustment sequence and the fourteen-factor comparison, here.
