PETALING JAYA: The artery has been reopened. The procedure went well, follow-up results are reassuring, and the cardiologist says recovery is on track.
Yet the staircase that once required no thought can suddenly feel like a decision. A flight overseas, a return to exercise, or even an unfamiliar ache in the chest may raise the same unsettling question: what if it happens again?
For many heart-attack survivors, recovery does not end when the immediate danger has passed.
In 2020 and 2021, Malaysia recorded over 15,000 admissions for acute coronary syndrome, which includes heart attacks, according to the National Cardiovascular Disease Database. Behind those numbers are patients who must gradually learn to trust their bodies again.
Consultant cardiologist Dr Yew Kuan Leong said this process begins well before a person goes home.
“Recovery actually begins before the patient leaves the hospital,” he said. “It is not just about the angioplasty or stent procedure being successful, but also about what the patient does afterwards.”
That may involve medication, changes to diet and exercise, and cardiac rehabilitation where appropriate. How quickly someone returns to work, driving, travel and physical activity varies according to their condition and recovery.
But physical progress does not necessarily mean confidence returns at the same pace. Consultant cardiologist Dr Shathiskumar Govindaraju said a heart attack can leave some survivors fearful, irritable, low in mood or unable to sleep properly.
“A heart attack is not only a physical event, it can also be an emotional shock similar to a traumatic event,” he said. Some become acutely aware of every heartbeat or bodily sensation, while others worry constantly about another attack.
“Whenever I see a patient after a heart attack, I don’t just ask, ‘How is your chest?’ I also want to know, ‘How are you sleeping? How are you feeling? Are you afraid?’” Shathiskumar added.
That anxiety can make ordinary sensations suddenly seem ominous. Yew said some patients become worried by chest discomfort, headaches or numbness in a hand, fearing that each symptom signals another heart attack.
Not every sensation is heart-related, but patients should not attempt to diagnose themselves, either. New or worsening chest pain, breathlessness or fainting should always be medically assessed.
The doctors noted that some caution after a heart attack is useful. It can encourage people to take medication consistently, attend follow-ups and pay closer attention to their health.
The problem, Shathiskumar pointed out, lies in "whether fear is helping the patient protect their heart, or preventing them from living their life".
That may show up as repeatedly checking one’s pulse, constantly seeking reassurance or avoiding exercise, travel and social activities despite being medically cleared. Some patients may also become frightened of being alone.
Family members can help by taking those fears seriously rather than brushing them aside, while still encouraging the patient to follow medical advice and gradually resume everyday activities.
If anxiety, low mood or disturbed sleep persists for several weeks or starts interfering with normal life, patients should speak to a doctor. Persistent hopelessness, panic or thoughts of self-harm require prompt professional attention.
For those who have been cleared to become active again but remain hesitant, Yew recommends patience rather than either extreme. “Do not be afraid, but do be patient and take things one step at a time," he said.
Cardiac rehabilitation can be particularly useful because it provides a structured way to exercise, understand risk factors and regain confidence in what the body is capable of doing.
The good highway
There is also another misconception doctors frequently encounter: that once a blocked artery has been treated, the underlying heart disease has disappeared. It has not.
“I would describe angioplasty as creating a good ‘highway’ for the coronary artery, but the patient still has an important role in looking after that highway,” Yew said.
Angioplasty treats a blockage, but coronary artery disease can continue to progress. Medication, physical activity, healthier eating, not smoking and regular follow-up therefore remain important long after the procedure itself.
Treatment has also become more sophisticated as cardiologists increasingly encounter younger patients, more women, and complex disease involving multiple or heavily calcified arteries.
Newer imaging tools allow doctors to examine arteries more closely, while specialised techniques can help manage blockages that are more difficult to treat.
Even so, Shathiskumar said the measure of a successful recovery should extend beyond whether an artery has been reopened or a patient has survived the immediate event.
“We want them eventually to say, ‘I survived it, I recovered, and I’m living my life again.’”
