KUALA KANGSAR: Every morning, 75-year-old Jahara Hashim gets on her bicycle and rides for about 30 minutes from Kampung Changkat Manong to the Senior Citizens Activity Centre in Manong. She has diabetes and high blood pressure, and has lived alone since her husband died 22 years ago.
Her two children are married and have repeatedly asked her to move in with them, but Jahara prefers to remain in her own home.
What she does not want, however, is to spend her days there by herself.
For the past five years, she has arrived at the centre by 8am and usually returns home at about 1pm. The routine gives her somewhere to go, friends to talk to and, perhaps just as importantly, people who would notice if she suddenly stopped turning up.
“My children do worry about me. They tell me to just rest at home in case anything happens to me,” she told Bernama. “But staying at home can be boring.”
Her children call regularly and visit every week, while friends at the centre also know her habits well enough to spot when something is amiss. That combination of independence and connection is becoming increasingly important as Malaysia’s population ages.
When nobody notices
Cases in which people die alone at home and are discovered only days later have become increasingly visible in local news reports. The phenomenon is often associated with Japan, where the term "kodokushi", or “lonely death”, is commonly used for people who die alone and remain undiscovered for an extended period.
In Malaysia, the issue is not confined to the elderly. People in their 30s and 40s have also been found dead alone at home, according to the Perak fire and rescue department.
Its director, Sayani Saidon, said the department recorded 15 such cases in the state between 2021 and July this year. Four were recorded in 2025, with another four between January and July 2026.
In many of the cases, firefighters were called by police or members of the public to gain entry into locked homes. Some of the bodies had already begun decomposing, and several people were believed to have been dead for more than a week.
Sayani noted that more older people are now living alone as adult children move elsewhere for work or as families become smaller and more geographically dispersed.
The risk for them is not simply loneliness: a person who falls, becomes suddenly unwell or loses consciousness at home may not be able to call for help, particularly if nobody nearby is aware of their usual routine.
Why Perak is especially vulnerable
Perak has the highest proportion of residents aged 65 and above in Malaysia. According to the statistics department, 10.8% of the state’s 2.58 million residents are aged 65 and older, while 15.3% are 60 and above.
Those numbers matter because ageing can bring a greater risk of chronic illness, reduced mobility, falls and loss of muscle strength. None of these inevitably prevents someone from living independently, but they can make social contact and regular check-ins more important.
For many older Malaysians, living alone is also a choice rather than a sign of neglect. They may feel comfortable in familiar surroundings, value their independence or simply prefer the routines they have built over decades.
Jahara is one example. She does not want to leave her home, but she has created a daily rhythm that keeps her connected to others.
That distinction matters because the real vulnerability is not necessarily living alone, but living alone without anyone noticing a change in routine. Sayani said families who live far from ageing parents should maintain regular contact and know how to reach a nearby neighbour if something seems wrong.
Neighbours, meanwhile, can play a role simply by being aware of those around them. This can be especially important for older residents and people with chronic health problems who may need help quickly if something happens at home.
Staying connected - and physically capable
For older people, maintaining independence also depends partly on physical health. Perak’s senior-citizen programmes have begun paying closer attention to sarcopenia, the gradual loss of muscle mass, strength and function that can occur with age.
The condition can make ordinary tasks such as climbing stairs, carrying groceries or getting up after a fall increasingly difficult. Health screening, strength-building exercise and adequate nutrition can help older adults better understand and preserve their physical capabilities.
But physical fitness is only one part of healthy ageing. Regular activities outside the home can also provide structure, companionship and something less easily measured: the reassurance that somebody knows you are there.
For Jahara, that network exists in the simplest of ways. She rides her bicycle to the same place each morning, spends several hours among familiar faces and heads home again after lunch.
And if one morning she does not arrive, someone is likely to notice.
