La Rochelle public prosecutor Arnaud Laraize confirmed that an investigation had been opened after the 31-year-old South African rugby player was found unresponsive at the Lions' hotel on Île de Ré on Friday, 16 October. Emergency services were called to the venue shortly after 10:20am local time. A source close to the investigation told AFP that Andrews was in cardiac arrest when discovered.
The investigation has been assigned to the Saint-Martin-de-Ré gendarmerie. According to reports from the Associated Press and AFP, accidental death is the leading hypothesis, and investigators had not identified signs of third-party involvement in their initial updates.
What investigators found at the scene
French media reported that Andrews was found in a shower cubicle locked from the inside. Prosecutor Laraize told the Associated Press that the player had taken part in a hammam session roughly an hour before he was discovered, and that those around him had not noticed any signs of health problems in the preceding days.
These details form part of the publicly reported circumstances but do not, on their own, provide a medical explanation for his death.

Why the autopsy outcome is critical
SA Rugby attributed Andrews' death to a fall at the hotel. French authorities, however, launched an investigation specifically to establish the cause of death, with an autopsy ordered as a central part of that process.
The autopsy could help determine whether an injury, an underlying medical event, or a combination of factors contributed to his death. No findings from that process had been confirmed in public reports available on 17 October.
The accidental-death hypothesis reflects a preliminary investigative position rather than a final conclusion. French authorities had not publicly resolved the outstanding questions as of the latest available reporting.
Andrews died one day before the Lions were due to face Stade Rochelais in the Investec Champions Cup in France. The Johannesburg-based franchise confirmed the fixture would go ahead as a tribute to the player. The rugby community offered condolences to his wife, Maiglin, and their two sons, Hendrix and Emerson.
Andrews, who was 31, had joined the Lions ahead of the 2026/27 season after previous stints with the Sharks in South Africa and Sale Sharks in England.
