Presentation
A 53-year-old butcher visited the doctor for a painful red area that developed on his hand 3 days prior. He had no history of recent travel but had accidentally cut himself when processing meat. He appeared well and did not have systemic symptoms. On dermatologic examination, erythematous vesiculated papules and plaques and associated edema were seen on the hand and fingers. A gram stain performed on fluid from a vesicle revealed gram-positive rods. Can you diagnose the patient?
