Whether it’s getting ghosted after a date or realizing you weren’t invited to a friend’s party, rejection stings. But for some people, it hits way harder: sudden anger, tears, shame, or all-consuming emotional pain that feels impossible to get over.
Those people aren’t just “too sensitive” or “overly dramatic.” There’s a real term for what they’re experiencing: Rejection sensitive dysphoria (RSD). While RSD isn’t an official clinical diagnosis, that doesn’t make it any less real for people who live it, according to research in PLOS One.
RSD is often linked to attention deficit hyperactivity disorder (ADHD), but just about anyone can be affected by it. Ahead, psychology experts explain the science behind RSD, how to spot it, and how to cope with rejection.
Meet the experts: Sasha Hamdani, MD, is a psychiatrist, ADHD clinical specialist, and author of Too Sensitive: Rejection, Resilience, and the Science of Feeling Deeply. Janice Castro, PhD, is a California-based psychologist and founder of J.C. Psychological Services.
What Is Rejection Sensitive Dysphoria?
“RSD describes an intense emotional response to real or perceived rejection, criticism, or disappointment,” says Sasha Hamdani, MD, a psychiatrist, ADHD clinical specialist, and author of Too Sensitive: Rejection, Resilience, and the Science of Feeling Deeply. The reaction is often much bigger than the event that caused it. “A lot of people can recognize that their reaction is ‘disproportionate’ but still struggle to control its intensity,” Dr. Hamdani says.
When you have RSD, even a mild interaction like a friend not responding to a text can trigger overwhelming feelings of shame, sadness, anger, or inadequacy, Dr. Hamdani says. Some people may feel hurt if their friend didn’t respond to a text, while others may not even think about it. A person with RSD might feel all-consuming emotional pain or distress that seems unbearable as they wait for a reply.
RSD isn’t recognized in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5), the official manual psychologists use to diagnose people with mental health conditions. That just means that it’s not an official diagnosis—but experts still find it important to recognize and understand. Janice Castro, PhD, a California-based psychologist and founder of J.C. Psychological Services, has seen RSD in her own work with clients—especially those with ADHD—and says that more therapists are starting to use the term. “Just because science hasn’t caught up yet doesn’t mean the experience isn’t real,” she says.
The RSD conversation is important because it gives many people a name for an experience they previously didn’t have words for, Castro says. The more therapists and the general public talk about RSD, the closer we get to having more tools and strategies people can use to navigate their experience. “For those of us who have loved ones that experience RSD, we can also start having open conversations about how to support them—and maybe have a little more empathy rather than judge or criticize.”
Why Does Rejection Hit So Hard for Some People?
You can thank evolution. “If you think back to the Stone Age, for example, people were just trying to survive in community, and the idea of not having social connection or social bonds was a threat to survival,” says Castro. Nowadays, your body can react to rejection on a visceral level, as if you were in danger, and the surge of stress hormones can send you into fight-or-flight mode.
From a neuroscience perspective, rejection can overwhelm the brain. The amygdala (which helps detect potential threats) activates a stress response, while the prefrontal cortex (responsible for emotional regulation) may have difficulty putting the situation into context, Dr. Hamdani explains. “Essentially, the emotional response can happen faster than the brain can logically navigate,” she says.
Still, it’s unclear what exactly causes RSD. But if you deal with anxiety, depression, or naturally engage in a lot of self-criticism, you may be more likely to experience it, says Castro. Genetics, environmental context, relationships, and family history are important considerations, too, she says.
These Conditions Are Most Linked with RSD
Anecdotally, RSD is often associated with neurodivergence—which can include ADHD, autism spectrum disorder, dyslexia, and dyspraxia, to name a few—but more research is needed to understand how common it is.
RSD is most often talked about in relation to ADHD. People with ADHD can feel things intensely and quickly, and react impulsively, says Castro. If they feel rejected or excluded while already struggling to regulate their emotions, they may react in a way that’s disproportionate to what happened, she explains. This is one reason RSD is so often linked to ADHD. However, RSD isn’t exclusive to ADHD, Dr. Hamdani says. “People with anxiety, trauma histories, or other mental health conditions can also experience similar patterns.”
One example is social anxiety disorder. “There’s often an intense fear that you’re going to be rejected—or that something you say or do is going to be extremely embarrassing,” Castro says. You might avoid certain people or settings to evade potential discomfort. RSD is also common for people with borderline personality disorder (BPD) since it can involve deep fear of abandonment and criticism, Castro says.
You also might be more sensitive to rejection if you have obsessive-compulsive disorder (OCD), especially if you tend to ruminate on social situations. It can be an exhausting cycle: “You’re constantly thinking about a conversation, what you said, how it landed, then criticizing yourself and seeking reassurance,” Castro says.
Plus, people with a history of trauma can be extremely aware of social cues and, as a result, rejection. They might constantly wonder if others approve of them or fear upsetting others, Castro says. If they’ve had a traumatic experience, their nervous system is likely heightened, making them more sensitive to negative things happening again, she explains.
Signs of Rejection Sensitive Dysphoria
Read on for symptoms of RSD, per Dr. Hamdani. Remember: Anyone can experience a combination of any of the below emotions after being rejected (it stings, no matter what!). But, if you’re feeling these consistently and intensely, that could be RSD.
Emotional:
- Intense shame
- Sudden sadness
- Anger or rage
- Embarrassment
- Feeling worthless
- Self-criticism
Mental:
- Replaying or ruminating on past conversations
- Overanalyzing others’ tone and behaviors
- Assuming others are upset with you
- Catastrophizing (thinking the worst possible outcome will happen)
Physical:
- Rapid heart rate
- Stomach discomfort
- Chest tightness
- Crying
Behavioral:
- People-pleasing behaviors
- Perfectionism
- Avoiding situations where failure is possible
- Becoming defensive when receiving feedback
How to Cope with Rejection Sensitive Dysphoria
RSD looks different for everyone, and coping skills will depend on the individual. That said, here are a few recommendations for navigating RSD, whether you have ADHD or not, per experts:
In the heat of the moment, pause before responding.
Dr. Hamdani recommends creating space between the rejection (or feeling of rejection) and your response. “This might mean taking a few slow breaths, physically stepping away from the situation, or asking yourself, ‘What do I actually know happened, and what am I assuming happened?’” she says.
To help with this, Castro recommends 4-7-8 breathing. “Breathe in through your nose for four seconds, hold your breath for seven seconds, then release through your mouth for eight seconds,” she says. Pretend you’re exhaling through a straw so the air comes out slowly. “It's not going to reduce the pain, and it's not going to make the rejection go away—but it will reduce the intensity of your emotion so that you can pause before reacting,” Castro says.
Practice self-compassion.
Self-compassion, the practice of being supportive toward yourself when you’re experiencing challenges, is an important part of coping with RSD. This is especially true if you have ADHD and were diagnosed in adulthood, since you’ve likely “faced ongoing judgment, criticism, and reprimanding for making mistakes [due to ADHD]” over the years, Castro says. “That’s decades of hearing messages that you aren’t good enough, which becomes internalized negative self-talk and self-imposed pressure,” she explains.
If you’re going through RSD, Castro recommends trying a self-compassion exercise from Kristin Neff, PhD: Ask yourself, How would you respond to a friend in this situation? “As humans, we are our own worst critics, yet the way we talk to our friends is often with kindness,” Castro says. “Why not try treating yourself like a good friend and see what happens?”
Manage your stress and anxiety by practicing self-care.
Whether you have ADHD or not, taking care of yourself is crucial—that way if and when RSD hits, you at least have a good foundation for well-being, which will help you cope more effectively. Dr. Castro recommends prioritizing sleep (around seven to nine hours, ideally), balanced nutrition, and hydration to help regulate your mood.
Stress and anxiety can make RSD even worse, per Cleveland Clinic, so it’s important to manage them effectively. If you have ADHD, setting reminders for yourself can be an important part of stress management. “People with ADHD put in a lot of work every day to keep up with structure and routines that most neurotypical-functioning adults perceive as ‘easy,’ or that neurotypical folks ‘just do,’” Castro says. Little reminders can help you feel less overwhelmed even if you don’t have ADHD, so don’t be afraid to put those alarm settings to good use.
Long-term, consider going to therapy.
Both cognitive behavioral therapy (CBT) and dialectical behavior therapy (DBT) can be helpful for RSD, Dr. Hamdani says. Working with a licensed therapist can help you “recognize rejection-related thought patterns, challenge your assumptions, and develop healthier emotional responses,” she says.
If RSD is causing you to avoid social outings or work meetings, or is otherwise affecting daily life, it’s time to talk with a professional, Castro says. Role-playing is one of her favorite tools since it lets clients practice how they want to respond in future scenarios.
Medication may help, too.
If you struggle with ADHD or any of the mental health conditions mentioned earlier—or if it feels like RSD is taking over your life—consider asking your doctor or psychiatrist if medication would be a good option. For people with ADHD, various ADHD medications can help with broader emotional regulation, but no medication is specifically FDA-approved for RSD and the research is limited, Dr. Hamdani says.
Rejection is never fun, and it’s normal to feel hurt. But if you’ve been told you’re “too sensitive,” know that your emotions are valid and there’s support out there. And to anyone who says you need to tone down your feelings, Dr. Hamdani would like a word: “The goal isn’t to become less sensitive, but to feel more in control.”
