The mid-air loss of a Challenger 604 aircraft door shortly after take-off from Nnamdi Azikiwe International Airport, Abuja, has renewed concerns about aircraft maintenance, safety mechanisms and accountability, as investigators seek answers, writes OLASUNKANMI AKINLOTAN
The Nigerian Safety Investigation Bureau has commenced an investigation into an incident involving a foreign-registered Challenger 604 aircraft operated by Triton Aviation, which lost its door shortly after departing the Nnamdi Azikiwe International Airport, Abuja, for Lagos.
According to a statement made available to The PUNCH by the NSIB on Friday, the aircraft, with registration number T7-SEI, departed Abuja at 16:12 UTC on Thursday. It was about three nautical miles from the airport and climbing through 3,500 feet when the crew declared a “mayday” following the separation of the door.
The aircraft returned to Abuja and landed safely on Runway 22 at 16:34 UTC, carrying nine people, including three crew members and six passengers.
The NSIB Director-General and Chief Executive Officer, Alex Badeh Jr, said the missing door had been recovered and secured at the bureau’s wreckage hangar.
“The Bureau has commenced its investigation to establish the circumstances surrounding the occurrence,” the statement read.
The incident reportedly prompted an emergency response at the Abuja airport, where fire and rescue personnel and medical teams were placed on standby ahead of the aircraft’s return.
The Nigerian Civil Aviation Authority also confirmed the occurrence, noting that the investigation fell under the purview of the NSIB.
Despite the safe landing, the incident raised serious questions about how an aircraft door could detach mid-flight and the measures authorities must take to prevent a recurrence.
Understanding the risks
Aircraft doors are critical safety components designed to withstand the forces encountered during flight. Depending on the aircraft’s design, their security relies on mechanisms such as latches, locking pins, hinges and other safeguards.
Cabin pressurisation adds another layer to the engineering challenge. At cruising altitudes, the pressure inside an aircraft is maintained at a level that allows passengers and crew to breathe comfortably. The difference between cabin pressure and the lower pressure outside places considerable stress on the aircraft’s structure.
Many passenger aircraft are fitted with plug-type doors, designed so that cabin pressure helps hold them securely against the fuselage. Other aircraft designs rely on alternative locking systems and safety features.
A door separation can therefore raise questions about the integrity of its locking system, the condition of its components, maintenance procedures and whether the aircraft’s safety mechanisms functioned as intended.
However, the precise cause of the Abuja incident remains unknown. Investigators must establish what happened before any definitive conclusion can be reached.
Lessons from history
Aviation history offers examples of structural failures and maintenance errors that have prompted safety improvements.
On 24 February 1989, United Airlines Flight 811, a Boeing 747 departing Honolulu, Hawaii, suffered a forward cargo door failure at about 22,000 feet. The door separated from the aircraft, damaging the fuselage and causing a section of the passenger cabin floor to be torn away. Nine passengers died.
The investigation led to renewed scrutiny of cargo-door locking mechanisms and contributed to safety changes intended to prevent similar failures.
Another incident occurred in June 1990, when British Airways Flight 5390 suffered a cockpit windscreen detachment at 17,300 feet. The aircraft commander was partially sucked out of the cockpit, while crew members held on to him as the co-pilot made an emergency descent.
Investigators traced the failure to undersized bolts installed during maintenance. The incident underscored the importance of proper maintenance procedures, correct components and rigorous checks of critical aircraft systems.
Although these cases differed from the Abuja occurrence, they demonstrate why investigators must examine both mechanical systems and human factors when structural components fail.
Experts speak
Former rector of the Nigerian College of Aviation Technology, Zaria, Samuel Caukcrik, described the Abuja incident as serious, stressing the need to establish how the door became detached.
“It is serious, and should not have happened. The NCAA has limited influence over the aircraft as it is registered in San Marino. The NSIB is investigating the incident,” he said.
Caukcrik explained that the aircraft’s door design was an important consideration in understanding the occurrence.
“The door is an ‘outward-opening’ door and not the plug-in-type door found on bigger jets where the cabin pressure differential with the outside atmospheric pressure keeps the door firmly closed until landing even if the handle is not in the closed position,” he said.
He added that the aircraft should have safety provisions to prevent cabin pressure from forcing the door outwards.
“However, there is a ‘fail-safe’ on the aircraft with this type of door by not allowing the pressure in the cabin to build up to push the door outwards, as happened. The report will shed more light,” Caukcrik stated.
His remarks underscore the need to determine whether the aircraft’s safety features and door-locking systems worked as intended. Investigators must also establish whether maintenance lapses or other factors contributed to the door’s detachment.
Another aviation industry expert, Olumide Ohunayo, identified manufacturing defects, improper latching and maintenance errors as possible causes of door separation.
“Generally, for a door to pull out, there are three possible causes. The first is a manufacturing defect; the door may not have been properly latching, or during maintenance, some screws may not have been properly returned,” he said.
Ohunayo, however, cautioned against drawing conclusions before investigators complete their work, saying, “But in the case of this aircraft, which has been flown for about 21 years, this incident cannot be a manufacturing defect. We must also thank God that they hadn’t really gone up high, as that may have been catastrophic. Let’s await the investigators’ report so we don’t rush to a conclusion.”
He stressed that the industry’s priority should be to identify the cause and prevent a recurrence.
“Things happen in the industry, but what is important is to ensure that the cause is ascertained and that such an incident does not happen again. That is why we must wait for the investigators,” Ohunayo added.
Beyond safe landing
Experts stressed that the Challenger 604’s safe return and the absence of reported deaths should not downplay the gravity of the incident. Although the emergency landing was a positive outcome, it did not explain why a vital aircraft component detached mid-flight.
The aircraft’s San Marino registration also highlights the need for effective coordination among relevant aviation regulators, the operator and investigators.
While the NSIB leads the investigation in Nigeria, effective cooperation with the relevant foreign authorities may be necessary to establish the full circumstances.
For Nigerian aviation authorities, the priority must be to ensure that the investigation is thorough, its findings are communicated clearly and any necessary safety recommendations are implemented.
Ohunayo noted that door separation was not a regular occurrence, although similar incidents had happened in the United States in 2024 and elsewhere in the past. He maintained that the industry must address the risks to prevent a recurrence.
The Abuja incident underscores that aviation safety requires more than an effective emergency response; it also demands identifying and addressing the underlying causes of such failures. The NSIB’s findings will be essential in establishing what went wrong and recommending appropriate corrective measures.
Until the investigation is concluded, attention must remain on evidence, accountability and prevention. While the aircraft’s safe landing was reassuring, the aviation industry’s greater responsibility is to ensure that similar incidents do not recur.
