Thousands of athletes are set to embark on the Nationwide Children's Hospital Columbus Marathon this weekend. The 46th annual marathon and expo returns to Columbus Oct. 16-18. But there's a reason why Columbus Marathon 2026 is being called America's most meaningful race. This is because of the marathon’s unusual connection to children receiving treatment at Nationwide Children’s Hospital in Columbus, Ohio. The event has raised more than $15 million for the hospital since 2012.
"24 miles are represented by a patient at Nationwide Children’s, one is an inspirational mile for the special children who have been taken too soon, and one is an “alumni mile” for Marathon Mile Champions from past years. These patients and their families will be beside every mile of the course to cheer on participants and serve as a visual reminder of how you, along with the marathon, are helping ill and injured children through your participation," according to the official website.
The event also has a significant fundraising record. In 2025, the marathon raised more than $1.4 million for the hospital, taking the total raised since 2012 to more than $15 million. The money supports the hospital’s work with children and families who need medical care. This year marks the 46th time the marathon has been held, the 20th half marathon and the fifth for the Jesse Owens 5K and 1 Mile races.
Columbus Marathon is "America's most meaningful race"
The Nationwide Children’s Hospital is at the heart of the Columbus Marathon 2026, an event that brings thousands of runners to Columbus, Ohio, for a cause that goes beyond running. The 2026 Nationwide Children’s Hospital Columbus Marathon and Half Marathon took place on October 18, with more than 15,000 runners and walkers registered for the sold-out event. The marathon course covers 26.2 miles, and 24 of those miles are represented by patients from Nationwide Children’s Hospital.
According to the marathon’s official website, 24 miles are represented by hospital patients, while another mile is for children who have died and a further mile celebrates former patient champions. These children and their families are present along the route, cheering runners and sharing their stories.
The idea allows runners to see the people who benefit from the event’s fundraising efforts. It also gives spectators a chance to support families whose lives have been affected by serious childhood illnesses and injuries.

How do children become part of the marathon?
The marathon features its “Marathon Mile Champions,” children who have received care at Nationwide Children’s Hospital. Their stories help runners understand the people behind the cause.
The race also includes an “Encore Mile”, which recognises former patient champions from previous years. Another section, known as the “Remembrance Mile”, honours children who have died. Families can gather along this part of the course to remember their children and celebrate the time they spent together.
The race is set for Sunday and partial and full closures will appear starting at the start line on Long Street through the 26.2 miles of the marathon course and the 13.1 miles of the 1/2 marathon course. Closures will be based upon expected arrival times of the first and last athlete.
