KATHMANDU, Oct 25: The catastrophic flood that struck the Nepal-China border on August 26 was not merely a mountain disaster but a warning about the growing threats to human health from environmental changes in the Himalaya, experts have said in a correspondence published in the medical journal The Lancet.
The disaster in the Rasuwa–Bhote Koshi corridor demonstrated how an extreme mountain event can destroy infrastructure, disrupt livelihoods and expose vulnerable communities to a range of health risks. Its consequences extend beyond immediate deaths and injuries to the safety of drinking water, access to healthcare, mental well-being and the resilience of communities dependent on fragile mountain ecosystems.
In their correspondence titled “China–Nepal border catastrophic floods: a planetary health alarm,” the experts argue that the disaster should be understood within the wider context of rapid climatic changes in the Hindu Kush Himalayan region, widely known as the Third Pole. The region is warming faster than the global average, exposing mountain environments to increasingly severe climatic extremes.
The analysis was led by Mandira Lamichhane Dhimal and Anthony Costello, among other experts. It highlights the need to treat climate-related mountain disasters as interconnected environmental and public health emergencies, rather than isolated events. The authors also call for international support to help vulnerable countries strengthen their capacity to respond to these growing risks.
A flood that unfolded in minutes
According to the correspondence, the August 26 disaster generated a catastrophic flood that travelled at a speed of 188 kilometres per hour from its source. The flood covered approximately 20 kilometres within seven to eight minutes before reaching the China–Nepal border at Rasuwagadhi.
The extremely short interval between the event's origin and its arrival at the border illustrates the difficulty of detecting and responding to sudden mountain hazards. Communities and border facilities in the path of such a fast-moving surge can have very little time to evacuate.
The authors also highlight the longstanding risks associated with glacial lake outburst floods (GLOFs), which occur when water stored in glacial lakes is suddenly released. Their correspondence notes that 47 glacial lakes at risk of expansion and causing outburst floods have been identified across the Hindu Kush Himalayan region, including 21 in Nepal.

These risks are particularly concerning because mountain hazards can have consequences far beyond their points of origin. Floodwaters, debris and sediment can damage settlements, transport routes, hydropower facilities and other infrastructure, disrupting access to essential services.
Why this is a public health emergency
The health implications of such disasters are not limited to immediate casualties. When floods destroy roads, bridges and communication networks, they can also isolate communities from hospitals, emergency medical services and supplies of essential medicines.
Damage to drinking-water and sanitation infrastructure can increase the risk of contamination and waterborne diseases. Displacement can leave families without adequate shelter, food and sanitation, while interruptions to healthcare can affect maternal care, childhood immunisation and the treatment of chronic illnesses.
Children, older people, people with disabilities and those living in remote settlements may face particularly serious difficulties during recovery.
The psychological toll can also be substantial. Survivors who lose relatives, homes and livelihoods may experience prolonged grief, anxiety and trauma. Children can be especially affected when schools close, familiar surroundings disappear or family members are separated.
These are potential public health consequences of catastrophic flooding; their extent in the Bhote Koshi disaster requires systematic assessment. The correspondence's planetary health framing underscores why recovery efforts must consider the broader effects of environmental disasters on human well-being.
The climate connection
The Hindu Kush Himalaya is a critical source of water for South Asia, supporting drinking-water supplies, agriculture, hydropower and ecosystems across several countries. Changes in glaciers, snow cover and mountain stability therefore have implications for water and energy security well beyond the high mountains.
Although the precise mechanisms behind individual disasters require scientific investigation, rising temperatures can alter glaciers, thaw permafrost and weaken mountain slopes. These processes can interact with geological conditions and other factors to produce dangerous cascading events.
The authors' warning is therefore broader than the immediate destruction caused by one flood. Environmental instability in the Himalaya can threaten public health, livelihoods and essential services, making climate adaptation a fundamental component of health protection.
A call for international support
The paper stresses that Nepal, as a low-income country, needs access to modern technology, technical expertise and financial support from high-income countries to address the growing risks.
Better monitoring of glaciers and potentially dangerous lakes, improved early-warning systems, stronger health services and resilient infrastructure are important elements of preparedness. Cross-border cooperation between Nepal and China is also essential because hazards originating in one country can rapidly affect communities and infrastructure in the other.
Regional collaboration should include the sharing of scientific data, coordinated risk assessments and clear emergency communication arrangements. Public health authorities also need to be integrated into disaster preparedness so that warnings can trigger medical response plans, protection of water supplies and support for displaced populations.
The authors argue that the Himalayan agenda deserves greater attention in international climate negotiations to protect vulnerable communities and safeguard public health, water security and energy supplies across South Asia.
The central message is that the health of Himalayan communities cannot be separated from the health of the ecosystems on which they depend. The Bhote Koshi disaster offers a stark reminder that protecting people in a warming mountain region requires action that goes beyond emergency relief to address environmental risks, strengthen public health systems and mobilise international cooperation.
